lint improvements

Many improvements (too many to list) have been made to lint in the past few
OpenBSD releases

Reduce false positives
Add additional lint comments to mark "known issues"
Improved duplicate warning handling
Additional checks 
Cleanup removing hacks that work around old versions of gcc and older C standards
